Did you know that many insurance companies pay for nutrition counseling? 

 

You will need to get a referral from your doctor for one of the following before the visit:

  • Obesity

  • Overweight

  • High cholesterol

  • High blood pressure 

  • Diabetes or prediabetes

  • Underweight

  • Humana (Medicare Only)

  • United Health Care

  • UMR

  • Medicare (for Diabetes and Chronic Kidney Disease 3-5 or transplant)

  • Superior Healthplan

Verifying Nutrition Benefits 

You are responsible for checking insurance coverage PRIOR to your visit. While many preventative diagnosis are covered, it does not guaranteed that your plan covers nutrition services. By booking an appointment, you agree for full financial responsibility of $90 for your nutrition visit in the event that your insurance does not cover nutrition services. 

 

To verify your nutrition coverage please call the member 800 number on the back of your card and ask to speak with a representative.

Question to Ask:

  1. Do I have nutrition services on my current insurance policy?

  2. Is my diagnosis covered? [You can get this diagnosis from your doctor]

  3. How many covered visits do I have per calendar year?

  4. What is considered a calendar year for my current policy?

  5. Do I have a co-pay or deductible for nutrition services?

  6. Is there an associated cost for me if I choose to have the appointment as a telehealth visit versus in person visit?​​You will be asked to pay any applicable co-pays and/or deductibles at the time of your visit. 

 

 In the event you do not have coverage, you are charged $90 for the nutrition session. You are responsible for the copays, deductible, and co-insurance. In the event your claim gets denied by your insurance company, your credit card will be charged for the nutrition visit.

HMO Plans

​

If you have an HMO plan, such as BCBS HMO, you are responsible for having a referral from your PCP before you can visit with us. You will NEED your referral for nutrition counseling from your PCP before your visit, if not, then you will be responsible for the $90 service/cancellation fee. The referral cannot be a Physician Assistant or Nurse Practitioner, it must be from the PCP. Please call you PCP's office and ask to fax a referral to (972) 767-3583.
